Hospital Discharge Planning
Coming home from the hospital can feel overwhelming — new medications, new instructions, follow-up appointments. I review your discharge instructions with you, organize your medications, confirm your follow-up care is in place, and make sure you know exactly what to do in your first days home.
Medication Review
I review all of your medications — prescriptions, over-the-counter, and supplements — checking for duplications, interactions, and anything that no longer serves you. You and your doctor receive a clear, organized list you can actually understand.
IV Therapy Administration
When your doctor orders IV therapy at home, it should be administered by a registered nurse. I provide skilled IV therapy administration in the comfort of your home, with careful monitoring from start to finish.
Wound Care
Proper wound care prevents infection and speeds healing. I provide professional wound assessment, cleaning, and dressing — and I watch for warning signs, so small problems never become big ones.
Self-Management Support
My goal is your independence. I teach you to manage your own conditions with confidence — understanding your medications, recognizing warning signs, and knowing exactly when to call your doctor.
Appointment Reminders
Never miss important care again. I keep track of your medical appointments, remind you before each one, and help you prepare the questions you want to ask your doctor.
